Program Summary:
St. David's Center is excited to have been selected as a provider of Consultation Services in support of the new consumer directed program, Community First Services and Supports (CFSS). This new program is replacing the PCA program over the course of the next 15 months. As the program rolls out, we expect to provide support to hundreds of people across the state of Minnesota.
St. David's Center's Consultation Services will:
· Provide education to people to help them make informed decisions about how to meet their needs using CFSS.
· The people using the program can choose to receive help writing their service delivery plan. The program reviews and offers guidance to ensure the plans only contain services that are allowed per state guidelines.
· This relationship-based program also provides ongoing support to the person throughout their plan year as requested and in certain situations completes a semi-annual review as required.
Position Overview:
· The Consultation Services Supervisor will work in partnership with program leadership to develop and implement the program and ensure St. David's Center's Consultation Services are provided in alignment with the requirements of MS 256B.85.
· The supervisor will oversee the program, staff and participants in a way that ensures the integrity and availability of the service for those who use it.
· The Consultation Service Team will work remotely, the supervisor will create a culture that supports a strong sense of team, facilitate collaboration and relationships for the continuity of the program implementation across Consultation Services Coordinator staff.
· The supervisor will ensure the participant, and their representative is successful in accessing the CFSS services they need through the development and implementation of policies and procedures related to utilizing the service.
· Measure customer satisfaction and respond to results to ensure continuous improvement of our Consultation Service. Ensure participant service-related rights are maintained. Regularly assess the service to ensure compliance with statute and the quality of the consultation services provided.
· Oversee Consultation Services program operations and supervision components consistent with ethical best practices and St. David's Center mission, vision and values which results in excellent services, compliance with the service model and achieving program goals.
· Act as the liaison between the Department of Human Service, Fiscal Management entities, CFSS Agency providers, program staff and participants of the service.
· Partner with the leadership of Community-based Disability Services in the development and management of the Consultation Services Program Budget which results in financial outcomes for the program that align with annual budgeted goal(s).

Schedule: Fully remote, available during program hours of operation, 8:00am to 5:00pm, Monday through Friday.
Compensation (Pay/Benefits)
Our employees are our most valuable asset; therefore, we offer competitive pay and benefits. The pay range for these positions starts at $60,702 - $65,199 a year for full time, depending on qualifications (i.e., licensure/certifications and experience). This salary reflects a 40-hour workweek, annualized. Any agreed-upon reduction in weekly hours will be reflected in salary commensurately. Please inquire with interest. Job Qualifications:
· Education: The Lead (Supervisor) must meet at least one of the following education requirements:
1) Be a Doctor of Medicine or osteopathy
2) Be a Registered nurse
3) Have a bachelor's degree or higher in one of the following fields:
o Occupational therapist,
o Occupational therapy assistant,
o Physical therapist,
o Physical therapy assistant,
o Psychologist,
o Social worker,
o Speech-language pathologist or audiologist,
o Professional recreation staff,
o Professional dietitian,
o Psychology,
o Sociology,
o Counseling,
o Special Education,
o Rehabilitation counseling,
o Other human services fields
4) Have at least one year experience working directly with people who have an intellectual disability or other developmental disability and have an associate's degree or higher in a discipline associated with at least one of the following fields of study:
o Human behavior (e.g. psychology, sociology, speech communication, gerontology),
o Human skills development (e.g., education, counseling, human development),
o Humans and their cultural behavior (e.g., anthropology),
o The human condition (e.g., literature, art),
o Any other study of services related to basic human care needs (e.g., rehabilitation counseling).
o The human condition (for example, literature or the arts)
· Knowledge: Self-directed principles and person-centered planning. Be knowledgeable of requirements in the collective bargaining agreement between the State of MN and Services Employees International Union (SEIU) Healthcare MN, Principles of self-determination (Freedom, Authority, Support and Responsibility) Mandated Reporting, Fraud, Waste and Abuse, Data Privacy, and the Americans with Disabilities Act (ADA)
· Work Experience: A minimum of two years of full-time experience in one or more of the following:
o Consumer Directed Community Support (CDCS) support planning, or support broker;
o Case management or care coordination in Alternative Care, DD Services, or
o Case Management services provided to a child with a severe emotional disturbance diagnosis or to an adult with serious and persistent mental illness.
o Have provided consumer education to participants using self-directed services under an FMS enrolled with the Minnesota Dept of Human Services.
o One year of full-time work experience providing direct services to persons with disabilities or persons over 65;
o Two to 5+ years of supervisory experience; 2 to 5+ years of program oversight.
o Experience with Department of Human Services regulation and licensing.
· Skills/Abilities: Be age 18 or older, clear a background study as required under Minn. Stat. Ch. 245C
