Position Title : 

Children's Mental Health Case Manager Lead I

Compensation (Pay/Benefits) Our employees are our most valuable asset; therefore, we offer competitive pay and benefits. The pay range for this position starts at $53,144 - 59,051.20 a year for full time, depending on qualifications (i.e., licensure and experience). This salary reflects a 40-hour workweek, annualized. Any agreed-upon reduction in weekly hours will be reflected in salary commensurately. Please inquire with interest. This position is benefits eligible, including health and dental insurance, health savings account with an employer contribution, flexible spending accounts, and retirement account with matching contribution. Schedule: 

  • 40 hours per week, M-F. This position requires flexibility to work past 5pm two evenings per week.

Location: 

  • Hybrid officing with most in-person work based in the community.

Program Summary: 

Children's Mental Health Case Management provides community-based guidance for residents of Hennepin County with children up to age 18 (or 21 if in high school) who experience a mental health diagnosis which significantly effects their life.

St. David's Center offers a unique opportunity for case managers to provide community-based support and advocacy for families who have a child with significant mental health needs. The case manager and family together assess a child's needs and develop a care plan. Case managers assist the family with locating, accessing and coordinating services or community-based supports. Case managers advocate for and assist with maintaining services with the goal of helping the child be their best at home, in school and in the community.

Position Overview:

  • All typical duties of a children's mental health case manager with a reduced caseload
  • Make initial contacts with prospective families to ensure their interest in the service, completed related documentation, and coordinate with Central Intake on client's readiness to be assigned a case manager including arranging for DA completion as needed.
  • Support/assist with training program staff in using agency documentation platforms to help them gain proficiency and meet program, county and agency requirements.
  • Develop in partnership with Program Director and Supervisor strategies that support staff in learning.
  • Meet monthly with Program Director and Supervisor related to Lead I job duties.

Job Qualifications:

Education: 

  • At a minimum, a Bachelor's degree in one of the behavioral sciences or related fields from an accredited college or university

Experience: 

  • Minimum of 2 years of experience as a Children's Mental Health Case Manager at St. David's Center.
  • Experience with families from diverse communities
  • Strong communication, leadership and collaboration skills
  • Preferred: Strong skills in documentation
  • Strong skills in electronic health records, Outlook and Word
