Program Summary:
St. David's Center is excited to have been selected as a provider of Consultation Services in support of the new consumer directed program, Community First Services and Supports (CFSS). This new program is replacing the PCA program over the course of the next 15 months. As the program rolls out, we expect to provide support to hundreds of people across the state of Minnesota.
St. David's Center's Consultation Services will:
- Provide education to people to help them make informed decisions about how to meet their needs using CFSS.
- The people using the program can choose to receive help writing their service delivery plan. The program reviews and offers guidance to ensure the plans only contain services that are allowed per state guidelines.
- This relationship-based program also provides ongoing support to the person throughout their plan year as requested and in certain situations completes a semi-annual review as required.
Position Overview:
Intake and Scheduling. Provides overall guidance for clients and families who are new referrals to St. David's Center's consultation Services which results in families feeling informed and welcomed.
- Answer incoming calls and emails related to CFSS and our consultation services within 24 hours.
- Provide overview of the service and coach the client, caregiver/family through the process of service determination, intake, and initial meeting. Schedule initial appointments for the Consultation Service Coordinator Team and/or communicate wait list status, ensuring client is aware of expectations of the process.
- Manages the collection and dissemination of client data through MnCHOICES as a Security Liaison.
- Work closely with program supervisor and Finance and billing staff to provide seamless customer service and communication on payer and funding source issues.
- Coordinate interpreters for appointments as needed.
- In collaboration with the Supervisor, assist with DHS surveys, data collection (such as enrollment or complaints) and communication of policy changes.
Caseload Coordination. When assigned a caseload, provide education and information and complete the tasks required of the consultation service for the people we support and/or their guardian/spouse. Ensure the participant and their representative is successful in accessing the CFSS services they need within the scope of Consultation Services.
- Educate the person about CFSS.
- Help the person write their CFSS service delivery plan, to the extent the person desires.
- Review the CFSS service delivery plan and submit it to the lead agency for approval.
- Offer guidance to the person on whether the CFSS service delivery plan is complete and only contains covered services.
- Provide the person with a list of CFSS provider agencies (if the person chooses the agency model) or FMS providers (if the person is purchasing goods and services and/or chooses the budget model).
- Respond to questions from the person throughout the year.
- Help the person change their service delivery plan, model and/or providers.
- Complete a semi-annual review if the person does not have a case manager/care coordinator and their spouse or parent (if a minor) serves as their worker.
- When a consultation services provider works with a person who uses the CFSS budget model, the consultation services provider is responsible to:
o Work with the FMS provider to provide the person with ongoing support to serve as the employer of their workers.
o Check in with the person ensure they are completing employer tasks, such as:
? Ensuring the workers are competent to meet the person's needs.
? Orienting and training workers.
? Evaluating workers within 30 days of hire, the start of a new plan year or after a change in condition.
? Verifying and maintaining evidence of support worker competency, including documentation.
? Completing worker performance reviews at least once per year.
? Answering the person's questions during check-ins.
? Support the person's success with the budget model, offer assistance. In consultation with Supervisor discuss concerns that could lead to an involuntary exit.
- Maintain and meet documentation requirements.

Schedule: The Consultation Service Team will work remotely, however there will be in-person agency and program meetings as well as the need to send mailings from a St. David's Center location. Must be available during program hours of operation, 8:00am to 5:00pm, Monday through Friday.
Compensation (Pay/Benefits)
Our employees are our most valuable asset; therefore, we offer competitive pay and benefits. The pay range for these positions starts at $49,870 - $59,045 a year for full time, depending on qualifications (i.e., licensure and experience). This salary reflects a 40-hour workweek, annualized. Any agreed-upon reduction in weekly hours will be reflected in salary commensurately. Please inquire with interest. Job Qualifications:
Education: Have a bachelor's degree in one of the following fields:
- Occupational therapist,
- Occupational therapy assistant,
- Physical therapist,
- Physical therapy assistant,
- Psychologist,
- Social worker,
- Speech-language pathologist or audiologist,
- Professional recreation staff,
- Professional dietitian,
- Psychology,
- Sociology,
- Counseling,
- Special Education,
- Rehabilitation counseling,
- Other human services fields
Or, have at least one year experience working directly with people who have an intellectual disability or other developmental disability and have an associate's degree or higher in a discipline associated with at least one of the following fields of study:
- Human behavior (e.g. psychology, sociology, speech communication, gerontology),
- Human skills development (e.g., education, counseling, human development),
- Humans and their cultural behavior (e.g., anthropology),
- The human condition (e.g., literature, art),
- Any other study of services related to basic human care needs (e.g., rehabilitation counseling).
- The human condition (for example, literature or the arts)
Knowledge: Self-directed principles and person-centered planning. Case Management or social and health services to individuals with developmental disabilities. Knowledge of community resources and activities. The ability to learn procedures regarding funding sources and service regulations. Principles of self-determination (Freedom, Authority, Support and Responsibility) Mandated Reporting, Fraud, Waste and Abuse, Data Privacy, and the Americans with Disabilities Act (ADA)
Work Experience: Two years of full-time work experience providing direct services to persons with disabilities or persons over 65; or experience coordinating or directing services for persons with disabilities or persons over 65 including self-directed services or experience coordinating your own services is required. One year experience in case management or coordination of services for individuals/families with developmental disabilities is preferred; preference includes experience with PCA, CSG, DD Waiver, developmental disabilities services; minimum of one year of supervisory experience preferred. One year of clinic or office operations preferred.
Skills/Abilities: Be age 18 or older, clear a background study as required under Minn. Stat. Ch. 245C. Highly developed customer service skills, leadership, excellent communication and problem-solving skills, organizational skills with the ability to prioritize and perform multiple tasks. Ability to communicate effectively, both through written and verbal communication. Ability to work autonomously while still placing value and importance on teamwork. Coordination of services and information. Strong professional writing skills. Clinical and/or person-centered planning/writing skills preferred. Mathematical skills, particularly with a focus on budget management, a plus. Proficiency with Excel, including formulas preferred
