Intake Coordinator

Program Summary:

St. David's Center is excited to have been selected as a provider of Consultation Services in support of the new consumer directed program, Community First Services and Supports (CFSS). This new program is replacing the PCA program over the course of the next 15 months. As the program rolls out, we expect to provide support to hundreds of people across the state of Minnesota.

St. David's Center's Consultation Services will:

  • Provide education to people to help them make informed decisions about how to meet their needs using CFSS.
  • The people using the program can choose to receive help writing their service delivery plan. The program reviews and offers guidance to ensure the plans only contain services that are allowed per state guidelines.
  • This relationship-based program also provides ongoing support to the person throughout their plan year as requested and in certain situations completes a semi-annual review as required.

Position Overview:

Provides overall guidance for clients and families who are new referrals to St. David's Center's consultation Services which results in families feeling informed and welcomed.

  • Answer incoming calls and emails related to CFSS and our consultation services within 24 hours.
  • Provide overview of the service and coach the client, caregiver/family through the process of service determination, intake, and initial meeting. Schedule initial appointments for the Consultation Service Coordinator Team and/or communicate wait list status, ensuring client is aware of expectations of the process.
  • Manages the collection and dissemination of client data through MnCHOICES as a Security Liaison.
  • Work closely with program supervisor and Finance and billing staff to provide seamless customer service and communication on payer and funding source issues.
  • Coordinate interpreters for appointments as needed.
  • In collaboration with the Supervisor, assist with DHS surveys, data collection (such as enrollment or complaints) and communication of policy changes.

Schedule:

The Consultation Service Team will work remotely, however there will be in-person agency and program meetings as well as the need to send mailings from a St. David's Center location. 32-40 hours a week Must be available during program hours of operation, 8:00am to 5:00pm, Monday through Friday.

Compensation:

The pay range for these positions starts at $20.64 - $22.93/hr annually, depending on qualifications (i.e., licensure and experience). Please inquire with interest. Job Qualifications:

Education:

An associate degree or higher in a discipline associated with at least one of the following fields of study: Human behavior, Human skill development, Humans and their cultural behavior, Human condition, or any other study of services related to basic human care needs. Will except equivalent experience in leu of a degree

Certification/Licenses:

The ability to learn procedures regarding funding sources and service regulations. Principles of self-determination (Freedom, Authority, Support and Responsibility) Mandated Reporting, Fraud, Waste and Abuse, Data Privacy, and the Americans with Disabilities Act (ADA)

Work Experience:

Required:

  • 1-2 years related experience.
  • Experience working independently, managing own tasks & time, yet functioning well in a team environment.
  • Experience that requires the ability to establish and maintain positive professional relationships, with both internal colleagues and external customers.
  • Experience that requires the use of clear and compassionate verbal communication skills.
  • Experience that requires excellent listening and coaching skills, along with customer service.
  • Experience related to using computer databases/systems to support workflow processes.

Preferred:

  • 3-5 years related experience
  • Experience collaborating with team members on a daily basis in order to do the best work possible.
  • Experience managing multiple & diverse job duties simultaneously and remaining calm & composed with demanding customers.
  • Experience with quickly engaging with others during phone and in person interactions (both customers and colleagues).
  • Experience with Microsoft Office 365 (Outlook, Word, Excel, SharePoint)

Skills/Abilities:

  • Be age 18 or older, clear a background study as required under Minn. Stat. Ch. 245C
  • Highly developed customer service skills, leadership, excellent communication and problem-solving skills, organizational skills with the ability to prioritize and perform multiple tasks. Ability to communicate effectively, both through written and verbal communication. Ability to work autonomously while still placing value and importance on teamwork. Coordination of services and information. Strong professional writing skills. Clinical and/or person-centered planning/writing skills preferred. Mathematical skills, particularly with a focus on budget management, a plus. Proficiency with Excel, including formulas preferred.
  • Pass the Department of Human Services background check or additional background checks as indicated.
